I was thinking of entering a new trial with Tamoxifen. It is a study where half the patients take 20mg of Tamoxifen a day, and the other half of the patients take 10mg every other day. They do not let you pick which you will be assigned to, but you can drop out anytime. This is to see if every other day is just as effective as every day higher dose, with fewer side effects. Something to look into.
