Hello Everyone! I have been following this thread and it has been very helpful. Here is my quick story and any comments or help would be very much appreciated. I was diagnosed with PCa jan of 2024, psa 8.5, Neg DRE, positive MRI for two lesions, biopsy done and found pirads 5/5 and 4/5. Gleason 3+4 and also found a 3+3. Done at Hopkins. PMSA PET scan showed containment to the prostate, Decipher test was a 0.34, so decision was made to go on AS. 6 months later psa now 9.3, another MRI ( MRI pretty much remained the same) was done and another biopsy is scheduled. Spoke with a Radiation Oncologist, because AS just scares the hell out of me. RO says they don't have MRIdian there, but they want to do SBRT without ADT. They have Cyberknife but do not use it for prostate, because to takes to long to use per patient? I have a scheduled appointment at Thomas Jefferson Univ hosp with a RO. They have MRIdian there also Cyberknife. I don't want RP for sure and I want to get some sort of treatment started. I am concerned about how Viewray went bankrupt and wonder if the quality is being maintained on the instrumentation? I'm trying to decide which modality to use, thats if I get to choose? I'm even wondering if I'm missing something here because of my tunnel vision? This whole prostate cancer thing has me upended and confused. So may options, treatments and too many Doctors to interface with. The only thing that im absolutely convinced of is that I don't want surgery or ADT. I think Hopkins might be TrueBeam but not sure. I'm very active at 68 and still play sports, so I want to get treatment and then clear my mind and get right back to life. I will admit that right now my life has become a roller coaster ride and I am suffering though it. Sorry for the long winded story, I just needed to get it out.

Hey, ViewRay went bankrupt But I believe their machines are now being serviced by Elekta? Bens1 knows all of this so he’s the one to ask.
MRIdian is better IMO due to less healthy tissue damage. Uses no fiducials (targets implanted in prostate) as does Cyberknife; instead it uses real time MRI to halt treatment if the target zone moves.
They are both excellent technologies, but one RO told me it MRI took too long; His hospital had Cyberknife so his opinion is suspect!
