I also took Nubeqa , have been on it for over 2 years. That combined with chemo back in 2022 brought my PSA below 1.0. But last year it started going back up. at 16.3 now. The cancer has become resistant of the treatment. Looking into more options SBRT or pluvicto theroapy.

After Darolutamide ask about Radium 223 and ten after a year Abiraterone with Predezone.

Last get PSMA PET scan and qualify for LU 177 now approved in many states and Canada

@sademotts I am sorry to hear that you are in the same situation as my dad. If you don't mind me asking, was your PSA rise gradual? Also, did you have any symptoms that accompanied the rise?

Have you had a PSMA PET scan to see if your cancer has spread outside the prostate? That is what can cause your PSA to rise so much. SBRT on metastasis found could reduce your PSA. If your cancer has spread only/mainly to the bone, then radium-223 Could also be useful as well.

Pluvicto is also a next step To try. You should be aware that is only really successful in 1/3 of the cases somewhat successful in 1/3 and not successful at all in 1/3.

One thing you could try is switching from Nubeqa to Zytiga. It may not be successful, but it could be worth a try.
