My husband has vascular dementia and is legally blind - he jumped out of bed (we have a door in the bedroom to the back garden), opened the door, and went outside at 3 AM, waking me up. When he came back in, I asked what was going on. He stated the TV was on (not on), and he fell asleep instantly. He also urinated outside. It appears to be a hallucination - no new meds, no unusual stress. He also moves his legs at night like he is running, which happens regularly. He is aware of his dementia getting worse.

Have you checked out a Parkinson's diagnosis? The REM sleep disorder (the running legs) is one of the first signs of it.
My husband to get up in the middle of the night and go outdoors. Once had to go to the hospital due to hyperthermia. I stopped it (called sundowning) by giving him 10 mg of melatonin at 4 pm and another 10 mg of melatonin at 9 pm. He never sun downed again except for one time my son forgot to give him the first dose until 6 pm.
