How did his colon find its way into the chest cavity before the many miles of small intestines that preceed it... as they are first in line below the pylorus? (I'm obviously not a doctor).

Is the larger stomach finding at all related to the hernia... or was just discovered while in for the hernia repair? How much has his food consumption increased thru the years? Was this surgeon used to seeing esophagectomy patients? Was the hernia repair successful... and has his digestion and pooping returned to normal? How old is he now?

If it is a diaphragmatic hernia, the transverse colon can get caught up in that.
