I paid for mine then fought the insurance company, I'm sure I posted on here somewhere. Eventually they paid for it.
If you do pay for it, make sure you negotiate with the facility, all their prices are negotiable, particularly if you're able to say "I'll pay you in full at time of service." I got mine down by about 1/3 or more.
