Born in 1944. I had radical prostatectomy Aug. 2018, Gleason 4+3, ex capsular, seminal glands removed and some lymph. Margins were clean but PSA rose in 2020 and I had beam radiation. PSA again rising (0.85 Dec 2024) and will likely do a PSMA scan, possibly leading to hormone therapy, as it is metastatic.
As to penis pump question, urologist mentioned in passing after 2018 surgery that it could help blood vessel recovery but nerve damage from surgery was probably permanent. Prescribed Viagra and Cialis for me to experiment with. Surgery had cost length and Cialis was only intermittent help, seldom getting penetration hardness. About 2020 I had decided improving blood in the pelvis was worth doing and I got a simple manual pump, (under $20 on EBAY) and began to use. Program was at least once a day, EVERY day from then until now.
Results built slowly and nerve damage meant the sensations were altered, less pleasurable and no possibility of ejaculations since no seminal glands. I have stayed with it because the physical effects have been remarkable.
After 4 years of almost daily use I have regained lost length and substantially gained girth. It is not painless since a very tight ring works best and heavy vacuum can be uncomfortable.
Tight ring and heavy vacuum produce for me a very rigid deeply purple state more than capable (as long as ring stays on) of sexual activity. It stays until the pressure ring is removed. Your partner would find it more than sufficient.
Permit me an old story of the English lord who, on waking up found unusually good morning wood. When his manservant/butler asked if The Lady should be alerted he replied, "no please just get me the baggy Tweed trousers, I think I will smuggle this one into town".
Hope this helps you work on the problem. If you treat it like an important project worth some personal pain/discomfort you can have better times I think.
PS: most come from China, are inexpensive but as of today (Feb 50) may be caught by new USPS ban on Chinese mailings to US.

John, I’m happy for you with the results you described. Had my surgery in 8/24 and 2 yrs younger than you , got a pump months ago with little luck and now going thru hormone treatments and radiation I get too frustrated to try the pump every day. The ring hurts and the few times I was hard enough for sex , the pleasure that I dream having again wasn’t there. I was lucky having sex several times a week for 40 yrs with a younger wife who would have been happy with once a week wanted to please me. Can’t help but think she is not suffering as I am . When treatments are over I will try anything and everything to feel normal once again including injections. Good luck
