Vitamin B6: pyridoxine hydrochloride vs. pyridoxal 5’-phosphate?

Posted by Ray Kemble @ray666, Sep 14, 2023

I had posted this earlier in the midst of a longish thread. As I've not had any responses, I thought I might post it as a topic all its own:

Good morning. Maybe someone following these posts about B6 can help me sort something out. I understand vitamin B6 is available as pyridoxine hydrochloride in multivitamins, vitamin B-complex, and vitamin B6 supplements. My neurologist tells me that pyridoxine hydrochloride in the form of vitamin B6, if taken in excess, can lead to toxicity with its various PN-like symptoms. He assures me, however, that the form of vitamin B6 that I’m taking in EB-N5, which is pyridoxal 5’-phosphate, is essentially different; pyridoxal 5’-phosphate comes with its own dosage guardrails, but not the same toxicity concerns that accompany pyridoxine hydrochloride. Have you met this “two forms of supplemental B6” issue in anything you’ve read or been told? I have found brief mention of it in only a few places. I’d love to get some clarity. –Ray (@ray666)

Hi Ray,

You might have already found more info on that.
Just in case : The pyridoxine hydrochloride is the Synthetic Vitamin B6.Synthetic Vitamin B6 – Pyridoxine hydrochloride comes from petroleum ester, hydrochloric acid, and formaldehyde. It isn’t readily absorbed or converted and has been shown to actually inhibit the action of natural B6 in the body. It also has side effects not normally found with natural food sources of this vitamin. It has been found that long-term use of high doses of synthetic Vitamin B6 can cause neurological problems such as tingling and numbness in the hands and feet due to nerve damage caused by an accumulation of unmetabolized molecules from supplements.

Natural sources of vitamin B6 is Pyridoxal 5 phosphate, it's the active form of vitamin B6 refer primarily to food products derived from animal or plant sources which contain active concentrations of this micronutrient including poultry, beef liver, eggs, bananas, avocados and potatoes, red bell pepper (raw), pasilla peppers (dried - from Mexico), leafy greens like spinach and kale, dried shiitake mushrooms, chickpeas, sunflower seeds, pistachio nut.
Foods High In Vitamin B6

Natural forms of vitamin B6 are more likely to retain its bioactive components since they haven’t undergone any chemical processing prior to ingestion.

Dr. Janine talks about whole food vs synthetic vitamins looking specifically at Vitamin B6.

As a new member they didn't let me provide some links that I wanted to post. Might repost this in a few days or so.

Hi John,
Thank you so much for sharing the links!
I don't have neuropathy, but have other minor health issues.

Hello, I have notes I made when researching "critical nutrients for nerve repair" in which I noted this recommendation:
B6 - Pyridoxal-5-Phosphate - 80-100 mg (the active form of vitamin B₆, is a coenzyme in a variety of enzymatic reactions). I believe it was from a doctor online. He said the more expensive forms of the B vitamins were more effective than the cheaper versions usually used in inexpensive multi-vitamins. Hope this helps.

Why supplement when nutritious whole foods are the best source of the vitamins and minerals that we need? So many supplements, as well as vitamins or minerals are removed from foods during the processing faze and then added back in the form of synthetics. Our bodies were not designed to know how to use and sometimes eliminate these fake sources/chemicals. Are you deficient due to what you are or are not eating?

Vitamin B6 is found in many foods, including fish, meat, vegetables, fruits, and nuts.
Fish
Salmon: A rich source of vitamin B6
Tuna: A high-protein food that's also a good source of vitamin B6
Meat
Beef: A good source of vitamin B6, especially beef liver and other organ meats
Poultry: A good source of vitamin B6
Vegetables
Potatoes: A good source of vitamin B6, especially sweet potatoes
Spinach: A good source of vitamin B6
Lentils: A good source of vitamin B6
Fruits
Bananas: A good source of vitamin B6
Other non-citrus fruits: A good source of vitamin B6
Nuts and seeds
Peanuts: A good source of vitamin B6
Other nuts and seeds: A natural alternative to vitamin B6 tablets
Other foods
Avocados: A good plant-based source of vitamin B6
Fortified cereals: A good source of vitamin B6
Chickpeas: A good source of vitamin B6
Eggs: A good source of vitamin B6
Milk: A good source of vitamin B6
Vitamin B6 is important for brain function, metabolism, and the development and maintenance of the central nervous system.

I take EB N5 as well. I have Sjogrens top of neuropathy. i tested toxic for B 6 (488!)
BUT... I do not think the toxicity was caused by this brand. i wad also taking vitamin gummies and I think the gummies did it????
I DONT KNOW. Someone please help! Neuropathy sucks and I was so stoked to find a product that could help. If I quit all other sources of B 6 eould it be OK to return to taling the EB N5????

Welcome @miacrab, If the EB N5 is helping your neuropathy symptoms and providing some sort of relief, I would keep taking it if it were me but I'm not a doctor. There are many other vitamins and sports drinks that contain a high level of B6 that I would avoid taking. Not sure how much B6 your gummies had but it might be good to find ones without B6.

Have you had a discussion with your doctor on the B6 levels?

Hi thanks for answering. Yes, now I avoid ALL forms of B 6 but not sure if the B 6 in EB N5 is the same? It is called P5P. I believe the Smarty Pants gummies and other supplements have caused the toxicity, However, I was told I would get relief from neuropathy from P5P in the food-medicine called EB N5 (It contains other nutrients)
Thank you for your caring words!

Good morning miacrab (@miacrab)

I'm the fellow who posted the original message way back in 2023. Since then, I've been gathering as much information as a layperson possibly can about the distinction between B6 pyridoxine hydrochloride and B6 pyridoxal phosphate (P5P). I've been doing this because, on the suggestion of my neurologist (who is also a physiatrist), I've been taking EB-N5 –– or, I should say, I had been taking EB-N5, now almost two years' worth. Only last month, this same neurologist asked if I might be willing to switch to a slightly different formulation: EB-M8. I've only started taking EB-M8, so I'm unable to give it a grade: 1 for it's doing me harm, 10 for it's the miracle cure we've all been waiting for. Right now, EB-M8 is a ?. I suspect it'll remain a ? for quite some time. As I written here before, with regard to EB-N5, it, too, remains a ?, but that is to say: it does not appear to have done me any harm. In fact, my PN (chronic idiopathis axonal polyneuropathy, or CIAP), whose chief symptom is poor balance (no pain), does not appear to have worsened in all these months, or if it has, it has worsened only microscopically.

My best wishes to you, miacrab, al to all PNers!

Thank you, Ray, for your arduous endeavors!
I am a lay person with a degree in Google.
I am just desperate to find something to help with small fiber neuropathy.
I haven't gotten answers on treatment and that's kinda what I'm lookin for.
Best of Luck! 😉
