To answer your question I suggest:
1. Review Mayo insurance website, not sure if your policy is Medicare, Medicare Advantage, HMO, or private contracted insurance, you find helpful information there:

2. Call the number on back of your insurance card and ask insurance company if accepted at Mayo AZ
3. If still not sure, the web site above has phone numbers you can contact.

When talking to insurance company, make sure they know it is Mayo AZ you're asking about. Each Mayo locations has its own contracts with insurance companies, it varies between locations. Also, insurance contracts can change, so if took 5 years ago, does not guarantee they still take.
