Have erection but can't have an orgasm

71, SBRT completed last July, 4 months of Orgovyx. Fortunately, PSA remains undetectable. Testosterone back to my normal limits. Using Trimix now and I can get a solid erection but just don't seem to have the sensation I use to and although pleasurable, it's not intense enough to get me to an orgasm. I keep hoping it's going to return but wonder if it's ever going to happen. I expect some nerve damage from the radiation but radiation oncologist isn't certain. Anyone with a similar story? It's really getting me down. Thanks!

Are you having a dry orgasam?
I too had radiation & hormone therapy. I have a dry orgasam.
I orgasam, but nothing comes out. Is that what happens to you ?

I'm also 71 and I had NS RARP in June 2024. At this point in my recovery I can get an erection (with the aid of the pills) that's (just barely) good enough for intercourse but I also have reduced sensation in my penis. I'm not an expert on all this, but from what I've read the orgasm itself is mostly mental. That understanding helped me and my wife to modify our sexual encounters to emphasize the mental side more. Bottom line, it took some effort but I now regularly orgasm when having intercourse with my wife. But unlike before where it was primarily physical, now the mental/emotional side is more important. Also, we emphasize other erogenous zones (rather than my less sensitive penis) for the physical part. Also, I found my self confidence was a big factor in getting to orgasm. If I was worried about it, it usually didn't happen. But then I turned a corner and now I'm much more relaxed, which I believe greatly facilitates having an orgasm. Best wishes figuring out what works for you. Here's a video by a doctor at Sloan Memorial that's the best I found discussing the mental side of sex after prostate cancer:

I went to see Dr. Mulhall in New York about this exact issue. I lost the ability to have orgasms after four months of ADT (Orgovyx). He said that is an EXTREMELY common side effect and almost every man will have it. The good news is I've only been off Orgovyx for three months and orgasms came back about a month after stopping.

No orgasm at all. I feel like I might be able to but then can never seem to get over the hump if you know what I mean.

How long were you on it? That could have something to do with it as well. The first couple times weren't anything to write home about but the quality and ease of reaching orgasm has steadily improved. Still not 100% there but I'd say I can reach orgasm 70-80% of the times that I want to.

It will be two years in May since ringing the bell. And two years since I last received any hormone shots. My testosterone is slowly starting to come back up. And at first, I had dry orgasims however of late I do ejaculate semen but no sperm. The dry orgasims were definitely a different experience.
