Welcome to Mayo Connect. Yes, you can complete the authorization form and return to Mayo.
The link below is form that is required if patient is an adult. Print, fill out, have patient signed and then mailed, fax or emailed back to Mayo. Fax and email will be fast way to authorize. You need one form for each person the patient is requesting to have access. This form then becomes part of the patient's medical records.
