Do any of you have experience with an Oncologist (PC expert) ?

I am a 67 yo, PSA9, 3+3=6, Decipher 91. I would like a PC Oncologist to help manage urologist and radiation oncologist. Has anyone found a good PC oncologist and if so what made them good ?

PC oncologist are called Genito Urinary Oncologists. Depending on you where you live, you can probably find one in your area, But good ones are not everywhere. Where do you live?

You could always go to Marina del Rey, California and see Dr. Mark Scholz. You can see videos of him talking about his treatments on YouTube. He’s very highly respected. He Give these talks at the prostate cancer research Institute (PCRI) meetings, which happen at least a couple of times a year.

Boston is a good place to get help. If you want to find a real good GU Oncologist you could come to an Ancan.org meeting next Tuesday at 6pm eastern and Rick has a list of doctors across the countr. Get there 10 minutes early.

Where do you live? I would first see what medical centers are close enough to drive. If none consider flying to get a world renown medical facility.

Normally a urologist will refer you to a R/O. At major centers like Mayo, UFHPTI, and other top notch medical facilities that specialise in prostate cancer, it's diagnosis, and treatments.

If were me I would definetely seek out a R/O if you are going to consider radiation treatments. You Gleason Score seems low if I read it right. Was your Decipher rated at low risk, intermediate risk, or high risk. I do not have experience with Decipher scores that us a number.

The mentors can probably supply you with web sites, and information on reaching out to top notch prostate cancer centers.

Thank you JC76. Gleason 3+3=6, PSA of 9 (quadrupled since 4/2019), Decipher is high risk- 99th %'ile in the context of a Gleason 3+3=6. I live near Portland,ME so Boston is nearby and Dana Farber has an office near here. So could be much worse but the PSA and Decipher are a bit worrisome and my PC needs to be addressed in the best possible way. Thanks !

With a PSA of 9 and a diagnosis of Gleason 3+3=6 and a Decipher of .91 (high risk), I'd first have a center of excellence corroborate the findings from your MRI which I assume you had. Second I'd have a center of excellence give you a second opinion on your biopsy pathology. It's critically important that these fundamental test results be accurate and correct. Otherwise it makes good decision making really difficult and risky. My first reaction is Gleason 6 is barely a cancer at all, at least a cancer of concern. It does not metastasize. With a Decipher of .91 that's high risk so maybe your risk of the Gleason 6 evolving into something greater is possible. I'd wonder if something was missed on your MRI? If it wasn't, maybe your first option is Active Surveillance. That's often the course of action with Gleason 6 until or if it evolves into a higher Gleason score.

Thanks for your helpful comments. I have had three MRI’s and four biopsies with an early biopsy second opinion from Johns Hopkins. I’ll get a second opinion from a COE on both the latest biopsy as well as the most recent MRI. Thanks again pdcar4756
