Reduce dose of exemestane
After 3 months of debilitating joint pain with exemestane, I was told to try another AI. I don’t believe it will be better. So I researched if different dosage could be as effective and I found that research: Alternative Dosing of Exemestane Before Surgery in Treating Postmenopausal Patients With Stage 0-II Estrogen Positive Breast Cancer . : Conclusions and Relevance: In this randomized clinical trial, exemestane, 25 mg, given 3 times weekly in compliant patients was noninferior to the once-daily dosage in decreasing serum estradiol. - I am going to try this instead of stopping completely AI

I saw a new medical oncologist today, about a big increase in tinnitus after starting first, Letrozole, then switching to Exemestane. Doc recommended either cutting Exemestane tablets in half or taking one every other day. The tablets are tiny (1/8 inch) so I'm going to try the alternate day method. I hope my tinnitus reduces.
Has anyone else had an increase in tinnitus on AIs?
I have heard many people have troubles with AIs. I am not one of them. Sounds like the right thing to do to see if you can stop or cut down the tinnitus. Our bodies react so differently to meds.
My tinnitus is caused by 7 yrs ago, an autoimmune sudden loss of hearing on L side. Letrozole made everything worse! Exemestane, every other day is so different/tolerable. Tinnitus the same. I’m trying to keep positive attitude and soldier through it.
Geez. I wish your dr could find an answer. Hopefully someone with this issue will post or one of the moderators will have some links.

I'm very relieved to hear that someone else had exactly my experience of a great increase in tinnitus with Letrozole. I switched to Exemestane daily, and my tinnitus decreased a little but not back to its usual level. I'll see Exemestane every other day helps. It definitely helped to see a different medical oncologist. The new medical oncologist even does research into AI dosing. So glad I made the change.
After chemo and radiation and 8 months after starting AIs and Prolia I developed knee pain that within a few months became debilitating. I started using collagen supplements. I started with gummies, then powder and am currently taking capsules. The results were amazing. I can't point a finger at AIs (I've been on all 3) or Prolia so I don't know which caused the pain originally. All I know is what worked for me. I'm 70 and do have some mild osteoarthritis and osteopenia.
Which collagen capsules are you taking now? I read that one could take between 2.5-15 g /day. I am 70 also. Thanks
I have tinnitus also, but had it before I started the Exemestane. Sometimes my ear seems to scream louder than usual, but I guess I just learned to live with it. Never even thought the AI had any connection. I will try taking it every other day and see what happens.
Although I had read about taking Exemestane every other day, I didn't do it until my new medical oncologist suggested it. May I suggest that you consult your doctor first? After all, every cancer & every person is different. Good luck!
