Incontinence Improvement with Pelvic PT?

Its only been since December 7 that I've been in PT and recently began tracking using the 24 hour pad test. Since February 11 the average is 444/24 hours. The 2 day moving average is 418/24 hours.

This looks like I'm far behind the curve of those who eventually made significant improvement.

I'm interested in other's comments about their rate of improvement over the initial months following RP.

I do my daily kegels, and other exercises from PT. No caffeine or alcohol. 73 years old. No problem prior to RP.

With a one hour voiding schedule 24 hour grams is declining at 8g per day. One hour voiding is pretty much socially awkward. Two to three hour voiding seems far into the future.

I’m happy to read so many positive posts bringing great emotional support that my team does not address.

One element not discussed, unless I’ve forgotten, is climacturia. If you can get by incontinence, which I’m struggling with, and ED then climacturia is an issue that doesn’t get much discussion. In my case the care team position is “ it happens, too bad.”

I have found no solution and it’s relationship devastating.

Since I began keeping records on December 7 I have done about 10,000 kegel squeezes building from 3 second hold to 15 though sometimes 20 and the longest I can hold is about 40 seconds.

Its important to use the correct technique and my PT gave an iPhone app with video demonstrations.

My PT has me voiding every hour to "train" my bladder to expect a duration. When this is "dry" I'm to move to 90 minutes or two hours. Dry at one hour is 70 or more days out without sudden improvement.

Being a compulsive numbers guy records are average daily grams and the three day moving average grams, both over a 24 hour period.

Three day running average: 342 started at 522 19 days ago.
Daily average: 406
Average grams per hour: 17
Highest 24 hour: 613, Lowest 213.
I use a postal scale.

I've tried to post the past 19 days showing a continuous improvement, albeit too slow in my view.

For my hoped for sudden improvement I am looking for certain food, acidic in particular, that may affect the bladder. Its a process of elimination. So far ID's, sugar. Apparently okay caffeine and red pasta sauce.

Being this quantitative is my way to avoid that hopeless feeling and becoming depressed with this aspect of recovery.

Anyway, this is what I'm doing. Do not stop kegels.

Has any one tried Emsella Chair ?
(I had HoLEP prostate reduction and still have stress incontinence after 12 months inspite regular kegels with a PT

Esmella Chair apparently does 10,000 kegels every session of 30 mins. Mayo clinic was trialing this with patients who suffered from stress and urge incontinence

Any feed back will be appreciated as I want to enroll with Emsella Chair next Month I don’t know whether it will help or worsen my stress incontinence. Tks. Merry Christmas and happy new year to all participants.

Mayo clinic was trialing this Chsir with patients who had stress and urge incontinence. Can someone From Mayo clinic comment on this trial.

My doctor scheduled me for 1 treatment with this chair before my surgery. Basically it triggers all of the muscles in your pelvic region over and over again I can see how it would help. There is a dial where you can adjust the level of the pulses. I had it all of the way up.
I am not sure I needed it since I never had any issues before and I could hold it for 14 hrs or so easily. And since my surgery I had very minor issues just for a day or two.

Mayo Clinic Connect (https://www.mayoclinic.org/zh-hans/tests-procedures/penis-pump/about/pac-20385225) published the "Penile Pump" and has been used clinically. One of its functions is to improve urinary incontinence after prostate cancer surgery. What I want to know is: What's the price? What is the postage from the United States to China?
Vacurect penis pumps are also produced in the United States, which work better, but are expensive.

I had RP surgery at age 65. Post-surgery, a PSMA PET scan showed cancer present in lymph nodes near the prostate fossa. They were removed 4 months after the RP, and a couple of months later I began 2 years of ADT and had 39 salvage radiation treatments. Kegels alone helped me get good (not perfect) control through the first year post-surgery. After that, urinary continence worsened. My urologist put me on low dose tadalafil and ordered pelvic floor PT. PT helped a good deal, but I still wore pads at night and shields often during the day. Since coming off ADT on schedule, my muscle tone has improved over all, and the incontinence was virtually gone; however, when I went off tadalafil last week, incontinence returned. I'm about to go back on it and keep those muscles juiced up for a while longer. I continue to practice pelvic floor PT at home. Key takeaway: ADT reduces muscle tone; you may therefore experience reduced control; you may regain control after you complete your course of ADT and muscle tone returns.
