I think this is why tests like the Decipher Score are so important. Gleason scores - especially those in the Intermediate Range - are just so nebulous. A 4+3 to one pathologist is a 3+4 to another ( happened to me) but determining the metastatic potential of these cells is really the issue. Once it is determined that your cancer is one that will spread, it’s a no-brainer
