Stage 4, Hormone Therapy-Resistant: What happens after Chemotherapy?

We went to the Oncologist last week. He said it was up to us whether we go for Chemo now or wait a few more months. Apalutamide and Eligard don't seem to be working any more. He was diagnosed April 2023.
My husband has no pain but the cancer is spreading into lymph glands and bones.
The Oncologist said is was 12 sessions of Chemo and it's done every 3 weeks. That's 9 months of feeling really awful after Chemo!!
Is that the standard? 9 months??
Then the Oncologist said if he has no pain, we could wait a few more months and re-do the CT scan and bone scan..
We thought the hormone treatment would last a few years, not just one year.
After Chemo, then what?? Does the cancer go away?

Speak to your doctor and request a hereditary Genetic test. It’s a pretty normal thing to have done. Is there any cancer in his family aunts, uncles, cousins parents, grandparents.

I was on Xtandi then Radium 223 each for a year and did 2 chemo's with Doxetel which caused fluid in lungs now since gone after 6 weeks of Prednisone . Prostate radiated in 2017 Spread to spinal bones only in 2020. Now spots in T 10 and L4 to 5. Just had a slip and fall on sidewalk ice at bank last week and fractured my L1
Plan is to do a CT-PSMA scan in Feb and see if I qualify for LU 177 now approved as standard of care in Ontario

Yes, that's a good idea. They gave me that test as a matter of course after diagnosis at an Ontario cancer centre, but it might not be something that every doctor orders.
No one can tell you how long you have. You may live long enough to have something else kill you. You don’t mention what drugs you are on. You don’t mention your biopsy results and other information like current PSA in this message. You may have done that in another set of messages, but there are so many people. It’s impossible to really keep up without the information being supplied In the current thread.
If you stopped taking Xtandi, And are now taking nothing, then you need to get on something right away to stop your cancer from spreading. Ask your doctor what he thinks. It sounds like you were not a good choice for taking a drug holiday. If you’re just on an ADT drug like Lupron You must be resistant and need Xtandi or Darolutamide (Nubeqa) which has fewer side effects
Can you have SBRT on those three new spots, or do you have so many that they do not want to do that?
Definitely get that PSMA PET scan before making any real decisions. If you can have SBRT on those new lesions, then you can put off Pluvicto.
I did Xtandi 2 years ago then Radium 223 Today I started Zytega or Arbitrone Then looking a 177
I had L2 radiated 3cm out in 2020 so caution on other radiation especially with fracture in L 1 caused by slip and fall on BMO sidewalk
Juts got PSA results 96.8 down from 181.6 a months ago Chemo worked
Are you taking something other than Zytiga? Your PSA sure went up high, Were you not checking it for a while? Did it jump a whole lot all at once? I can’t understand how it got so high without doing something. Are you working with an Oncologist or even a radiation oncologist?
Did you Stop taking Xtandi Because your PSA Started going up even though you were on it?
T10, L4 and L5 Aren’t near L1. Did someone tell you they couldn’t be radiated? What’s the radiation oncologist have to say?
PSA was going up so took me off Xtandi 2023 and moved to Radium 223 in 2024 then PSA up again
Did 2 chemo's and got fluid on lungs Just now PSA dropped to 96.8 from 181.6
Free PSA < .04 as always
Just started Abiraterone today plus prednisone 10 mg Doing PSMA CT scan in Feb to qualify for LU 177 now approved in Ontario
Still on Oxygen till lungs clear from reaction to chemo
No radiation was offered
