Try to get referred to a urologist for an artificial urinary sphincter.
That's what I'm being assessed for in a couple of months.
Reported 90% success rate. Look it up online. Sounds like the solution for both of us.

Here we do artificial urethral sphincter treatment is not safe, the longest is ten years, there are many twists and turns in the middle. I have done Kegel exercises for more than a year, and now I have done pelvic floor muscle exercises for nearly four months, but when I left the hospital, I leaked urine 100%, and now I am 20% restrained at night!
