I'm sorry that you did not hear back in the time period promised. Here's how it usually works.

Completing the online form is the first step in the appointment process. Next, a scheduler should call you at the time you requested on the form (within 3 business days). During that call the scheduler learns more about your primary concern and why you wish an appointment. They can also help decide which department you should first visit.

Have you spoken with a scheduler? Or are you further along in the process where the physician team is reviewing your medical records, history, etc.? This part can take longer than 3 business days.

I have not spoken to anyone regarding the current request that I completed 15 days ago. No one bothered to call or send an email.
