Observacion, prostatectomia o Radioterapia? AS, Surgery or Radiation?

Tengo 70 años, biopsia de prostata indica Gleason 3+3 bien definido en 5/12, (40%, 50%, 20%, 60%, 15%) todas en el lado izquierdo. En 2/12 permeacion perineural presente. El tacto rectal indico partes duras movibles y la biopsia contornos definidos. Diagnosticado desde 2020 con HPB Grado II por ecografias anuales y PSA a partir del 2020 de +/- 4.6 casi sin variacion en controles anuales. Ultimo examen en agosto 2024 de 4.64.
Tomografia pelvica y torax no indica extension de adenocarcinomas.
Unico sintoma dificultadades urinarias propias de HPB.
¿Se recomienda observacion, prostatectomia o radioterapia?.

I think the MRI is better than nothing at all. Decipher might still be available to you, have you researched if you can have your samples sent to a country that has it? Looking at my online claims, I can see that Decipher was $5,500 (covered by insurance), so it could be you will be out of pocket for part of that (since the insurance price is almost always twice or three times the cash price).

Si, lamentablemente no hay posibilidades. Voy a hacerme una nueva TC con contraste y estoy esperando el resultado del analisis de mis nucleos de mi biopsia por un segundo patologo y de un nuevo PSA para luego consultar con un oncologo y otro urologo. De alli ... a tomar la decision mas trascendental en mis 70 años de vida.
