I think that the dizziness could be contributed by any number of things. I’m not a coffee drinker but have had dizziness since I started taking Prednisone. My rheumatologist and cardiologist each recommended various test (including reviewing and reassessing my blood pressure medication) to insure that it was not the culprit but the dizziness did not dissipate. I did notice that it is slightly improved as I taper my Prednisone. Unfortunately, I have had to stall my Prednisone taper and add a biologic because I have had significant pain with my Prednisone tapering. Hopefully, we can all move through active PMR soon. I had dizzy spells coming off Prednisone when a biologic was added to see if it would help me taper off prednisone more easily. I was on three blood pressure medications to control my "labile" blood pressure which could be 210/110 sometimes and more normal other times. The problem with my blood pressure was that it wasn't "regulated" well.

Many of my problems were related to a low cortisol. Prednisone replaces the cortisol our adrenals produce. Cortisol regulates inflammation and other things including blood pressure. When we take Prednisone many things are destabilized until our bodies adjust to our Prednisone dose. When we taper our Prednisone dose everything is destabilized again.

When we try to taper off Prednisone completely everything is destabilized until our cortisol production resumes. This takes time and it doesn't happen immediately. The following link explains some of the things cortisol regulates.

The nice thing was when I got off Prednisone and my cortisol level improved with the addition of the biologic, I eventually didn't need any blood pressure medications anymore. I'm grateful Prednisone doesn't interfere with my cortisol levels anymore.
