Which doctor is best to treat Osteoporosis?

Posted by happy2023 @happy2023, Nov 18 3:14pm

After 2 spinal compression fractures, and a recent DEXA scan confirming Osteoporosis, my question is who is best to treat the Osteo, a rheumatologist or an endocrinologist?

I consider not asking or ordering a simple dexa scan in later years (60)s
is poor patient care. They ask blood work mammograms for women and prostate exams so why not a simple 15 min dexa scan?.

Extremely poor patient care; especially how easy and inexpensive a dexa scan is and the knowledge you get from it. We do a lot of screening tests (colonoscopy, mammograms, lipid panels, CBC, chest x-rays, etc.) and yet this is simply not done until you are "elderly" and have very little great choices on how to handle a newly diagnosed chronic - and possibly life altering - disease. Don't know what happens in other countries but unfortunately this makes me feel great distrust for the medical profession as a whole. I'll stop now but wow!

Talk about dexa scans. Would like to kick my Primary Care Dr. In.the teeth for Not ever ordering one and low and behold have multiple spine fractures.

@lilacs777 It might be good to keep in mind that Reclast and other bisphosphonates have a recommended limit of 3-5 years.

I was not able to do Reclast during my 5 years of letrozole even though I already had osteoporosis, because my endo was afraid of triggering afib (I already had it every once in awhile, and this risk has been disproven since) and kidney disease.

I did lose bone and did fracture- after treatment was done, mainly because COVID interfered with going on Tymlos in time. I did two years of Tymlos and my bones are now better than before cancer, but that may be an unusual gain.

I think the answer will depend on your DEXA scores. Doctors aren't prescribing for osteopenia anymore and some people aren't doing meds for early/mild osteoporosis. All the meds have time limits and they add up so that those of us who have done a lot of them run out!

The other concern with not doing Reclast or Zometa (both zoledronic acid) is that they are not thought to be beneficial for preventing spread of cancer. That is something to discuss with your oncologist.

In your last paragraph did you mean to type "not" thought to be beneficial? I thought it was the opposite. That it's used for metastatic breast cancer and to prevent recurrence.

@dlmdinia Oh of course! So sorry. Reclast and Zometa ARE thought to be beneficial for preventing spread of cancer.

I was using a keyboard and probably didn't have my glasses on 🙂 Apologies to all! Very important point and thanks for catching the typo.

You're welcome! It's amazing how many times I try to proofread before I post and still fail to catch errors. And autocorrect does not help by changing perfectly valid words to something else. In this comment as I was typing I notice the word "times" in the first line somehow got changed to "yikes". I probably should have left it to prove my point. 😃
