What to take for osteoporosis

On Letrozole. Dexa scan shows back to osteoporosis in my spine, other areas still osteopenia.

Oncologist wants me to go on something. I have read many of these posts and am scared about more repercussions from another drug.

Years ago (before BC) I was on fosomax. I think it gave me gerd. What is the lesser evil here ? I'm 69, trying to stay active and capable of watching my grandkids.

I do take Vitamin D and have for years. Generally 5000 units. (Now with k- know better do better)

Any guidance is appreciated.

After reading all of these comments, I am terrified. I had to come off of Prolia to have a bone graft and still waiting to see if bone graft took. Today is November 19th, 2024 and the last Prolia shot I had was December of 2023. Both my primary doctor and the oral surgeon who is also a medical doctor told me not to take prolia until all implants were in place. What should I do for my bones and are they actually going to crumble? What about osteonecrosis? Should I be concerned about this. I truly am scared.

@leeleevb so it's been almost a year since your last Prolia. It sounds like they did not want to "lock in" gains with Reclast due to it being anti-resorptive . Not sure.

Maybe you could have a DEXA and trabecular bone score test done, as well as blood tests for bone markers, to see where you are. It could be reassuring. If not, then you may need to talk to your endocrinologist about what med to go on to keep you safer. We don't know your original scores so if rebound took you back to scores that weren't too bad, maybe you are fine. Ask your doc!

not everyone experiences rebound fractures when quitting Prolia. Most fracture happen within the first year. And you're halfway through the danger zone. Have you consulted with the physician who prescribed Prolia.

Prolia. I would ask for bone markers. CTX and P1NP. And make a treatment decision depending upon those numbers. If CTX is not greatly elevated, you might begin Forteo. Forteo would speed the integration of the grafts as well as hastening bone growth into the hardware. Forteo is one of the two drugs prescribed for osteonecrosis.
If CTX is elevated, as you would expect following disuse of Prolia, I would start a bisphosphonate. You are unlikely to experience bone necrosis from a six month or year on a bisphosphonate. It is a risk factor you already have with prior Prolia use.
