Having caught your attention, Ion review of my medical records which I requested from my urologist, I came across an error. In a list was Alcohol Usage. It showed Beer, heavy. Wine, heavy. Liquor, ( if I remember correctly) heavy.
This was followed by a note that I had been counseled.
I don't drink. Don't like any of it. Beer looks so good on a Mojave hot day, but I always find it tastes nothing like it looks in the ads.
Question is, is there any need to request a correction? I am giving the records to a 2nd opinion doc. And I am concerned this could worm its way into other areas of my life. Tia.

Get your erroneous records corrected.
You never know what unintended consequences may result from those inaccuracies.

Now & then, I need to drink H2O if I feel a coming seizure or close to that. I drink alit of H2O during the day and work outside or in. I drink sweet-tee or a can of beer, but 12 years ago I had an accident on. My bicycle. So now I’m a TBI member from my daily - 3 times-day. And need my drinking water
I drink a lot of water daily. I urinate a lot. I’m still dehydrated. Am I doing something wrong?
I drink a lot during the day and yeas I had peeing for often during the day, but going ro places I drink less
I did get that corrected as well as reversing the frequentl urination comment.
I saw my 2nd opinion NO yesterday and got what I consider to be good news. First, my Gleason score is 3/4, not 4/3, so a minor change but to the good. She suggested active monitoring and scheduled a PSA test (none in records) and telemed appointment in 2 months. She emphasized this is slow growing cancer. I think, while she can't say it, she's thinking I'll die with it, not from it.

Check the side effects of all of your meds. My husband was taking two that could cause dehydration and we didn't realize it until he had a nasty fall and ended up in the ER.
Write a letter or call patient relations. They are amazing. I've used patient relations twice in to different healthcare systems. Their job is to listen to us and bring our problems to the correct unit
