I can stop drinking anytime I want!

Having caught your attention, Ion review of my medical records which I requested from my urologist, I came across an error. In a list was Alcohol Usage. It showed Beer, heavy. Wine, heavy. Liquor, ( if I remember correctly) heavy.
This was followed by a note that I had been counseled.
I don't drink. Don't like any of it. Beer looks so good on a Mojave hot day, but I always find it tastes nothing like it looks in the ads.
Question is, is there any need to request a correction? I am giving the records to a 2nd opinion doc. And I am concerned this could worm its way into other areas of my life. Tia.

If it's not accurate I would definitely not want it in my medical records. I would also follow up with them sooner than later.

I would definitely contact the provider who wrote that note in your records and ask why and ask it to be corrected. Did you fill out any questionnaire that asked about drinking that might of been entered wrong?

The reason I recommend it be corrected is that a provider in future may see that note and assume a medical problem is related to alcohol and not look at other reasons. It is important the information is correct. In my experience, only the provider who wrote the note has ability to correct.

Do you have a way of sending message to provider who wrote the note?

Yes, you absolutely want to correct it. This is why everyone should read all the reports of their medical visits and let them know of any inaccuracies.

Yes it needed to be corrected. That information could influence your treatment options and you will always be asked. About it. Inaccuracies should always be corrected.

Easier said than done. I've been fighting to convince hospital I didn't have a knee replacement while there. They keep saying “I says on the computer you did.” I Ask them why I don't have any scar on my knee, a question they never answered. I have talked to medical records, my doctor, the hospital radiologist, the hospitalist and the doctor of the ladies medical records that got mixed up with mine.
Another hospital claims I have a depression problem. The problem seems to be that no one is willing to admit, they made a mistake. If it's on a computer it seems to be gospel.
Good luck with getting your records changed.
I hope the people you deal with are more reasonable than the ones I dealt with.

Everything else is okay. But at some point it gets into the medical descriptions and of course I can't understand most of it. I do intend to bring this up with my 2nd opinion visit.
