Anyone had salvage radiation treatment followed by Xofigo for mets?

My husband has had a prostatectomy and is taking Nubeqa and Orgovyx, but PSA is on the rise and Dr. has recommended salvage radiation to prostate bed to be followed with Xofigo for bone mets. Does anyone have experience with either? The treatment sounds promising.

I have had exactly what your doctor is recommending. Once the PSA hits .2 after a prostatectomy, they really need to do salvage radiation to try and kill whatever residual cancer is in the area of the prostate bed I had seven weeks of radiation 3 1/2 years after my surgery when my PSA started rising. I had no side effects from that treatment. Some people get a little fatigue after the sessions. I never had that. My PSA stayed undetectable for 2 1/2 years after having radiation. Results vary some people are completely cured by the radiation. Your Gleason score gives you an idea about whether or not it will come back.

I’m surprised they are starting with Nubeqa. I was on Orgovyx and nubeqa for over a year and it works very well. The thing is they normally start you on Zytiga and when it fails, move you to Nubeqa. That gives a longer path to recurrence. One reason they would not put you on Zytiga is that you have heart issues, in that case Zytiga is not a good drug for you.

One advantage of Nubeqa is that has very few side effects.

I’m getting a Zometa infusion in a couple of days. It’s works like xofigo. These are bone straightening agents. They don’t really prevent mets. Orgovyx will weaken bones and cause osteoporosis. You want to take these drugs to prevent it? Xofigo has a slightly higher risk of osteonecrosis, of the jaw, than zofigo, which is why my oncologist prefers it. When I first started taking Lupron a drug just like Orgovyx my doctor put me on Fosamax, which works almost the same as Xofigo. It’s a pill you take once a week. I saw a seminar a couple of weeks ago where a doctor discussed problems with the bones caused by Lupron and he emphasizes that it is essential to take drugs like Xofigo, fosamax or Zometa to prevent bone weakening.

Orgovyx has a lot of side effects. Hot flashes, brain fog, fatigue and more. If you have issues, come back with questions and people can help..

8 years ago radiated prostate out 4 years ago mets to L2 lumbar and radiated 1 inch out. Then on Xtandi, then Radium 223 or Xofigio. 3 year slater did 2 chemo's Had PSA flair google that up to 700 More important got fluid on lower lungs which may take weeks to clear using prednisone 80mg and lysak Also chemo gave me small embolism . On med for it too
Take 600 calcium and K2 + D 3 daily Also get on Xgeva 12 week injections to push calcium into pot holes where cancer is killed
I get Zolodex as well 80 and feel great Lost 38 lbs. as no chardonnay for 4 months 🙂 189 from 226

Thank you jeffmarc! I'm glad to hear about your experience with the SRT. He has had SBRT to a few areas in bone which are now non-detectable on PSMA PET scan, but 3 new areas have popped up as well as in the prostate bed. We decided against Zytiga due to side effects/health issues and he was started on Orgovyx. That worked well for about a year, but PSA started rising again, so Nubeqa was added. Now new areas in bone and prostate bed, so he's been recommended SRT, possibly Xofigo after the SRT. Best wishes to you!

@sh68, how are you and your husband doing? Has he started salvage radiation?

No, probably in January, maybe late December. Thanks for asking!
