Gleason 6 Decipher 0.64. Photon, proton, AS, TULSA?

Posted by ksellers3 @ksellers3, Oct 3, 2024

I was recently diagnosed with above numbers. Is HIFU and TULSA the same thing? Wry confusing trying to decide which treatment I should try. Thoughts?

HIFU and Tulsa Pro are both High Intensity Ultrasound, but, Tulsa is much more precise. It is done in an MRI and exact location and temperature is monitored in real time. I had Tulsa on July 17 at Mayo in Rochester. Full writeup of of my experience: https://connect.mayoclinic.org/discussion/tulsa-pro-experience-mayo-clinic-mn-july-2024/

Medicare and supplement covered all but $6. I am part of their clinical trial.

Dr. Woodrum will be presiding over my TULSA-PRO procedure on this coming Wednesday, Oct. 9. Hoping mine goes as smoothly as yours did. May I ask, how large was your prostate?

45cc. Good luck. I had a reminder in my phone to send you a note next week. You are in good hands!

I'm curious. I'm about to undergo TULSA-PRO for one small tumor. G7 (3+4) less than 10% 4. Decipher Biopsy score 0.78. No cribriform. No other tumors visible on MRI. Did your biopsy not indicate cribriform? I had 2cnd and 3rd reads done on my biopsy samples at both Johns Hopkins and Mayo Rochester. Neither indicated cribriform. I'm wondering why your biopsy results didn't show that fact. For me, I'd be doing AS if it weren't for the Decipher score.

Hey @pdcar4756
I am G7 (3+4). one tumor. 25% G4. But High Decipher. .80. thinking of HIFU also. Why Tulsa and Not Focal One or SonaBlate?

I’m so confused. I thought Decipher test required a tissue sample after a prostatectomy from the tumor cells. How can one have a Decipher score then?

The Decipher test can be done on tissue from either a biopsy or prostatectomy.

Would the tissues still be there? I know the slides are.

The Decipher test can be done prior to surgery or after. The Decipher test when having radiation is done prior to treatments. The Decipher score gives your urologist and R/O a number the is more precise diagnosis of your cancer risk level.

Decipher test can help your medical providers give options on treatments. In my case my Gleason score of 7 (given an intermediate risk) was radiation and hormone. My Decipher came back low risk and my treatment changed to just radiation only.

It would have been beneficial for you to have that Decipher test done prior to your surgery. Most of the time the tissue taken for biopsies is large enough for Decipher to be done and no need to have another biopssy. That is how mine were done.

When you have surgery I am reading that surgeons now has access to your prostate and all areas of it to take another biopsy and get a Decipher score not just the ones at biopsies. If your cancer was still inside prostate this would be beneficial and confirm risk level. When it is known by other test your prostate cancer has spread out side your prostate the Decipher test is not valuable as your tests revealed already metasised and you would have been diagnosed with high risk.

I hoped this helped. It was confusing to me when I first got prostate cancer. I was not on MCC at that time. I began hearing Gleason Score, PSMA, Bone Scans, Decipher test, etc. and was overwhelming. My R/Os did their best to explain but it can be overwhelming.

I did not have RP just radiation but did have the Decipher test, PMSA, bone scan done after biopsies taken and prior to final diagnosis and treatment options given to me.

My Decipher was .78. My lesion was located on the anterior (front) side of my prostate. It could not be reached transrectally using other focal therapy modes/methods. I also liked TULSA-PRO because it was a treatment from the "inside to the outside" as opposed to from the "outside to the inside". Sending the high frequency ultrasound waves from the urethra to the prostate tissue as opposed to from the rectum into the prostate made more sense to me.

Thanks. I have not had any treatment yet so I’d have to have another biopsy for a Decipher test. I was ignorant and my urologist never mentioned it. With your Gleason 7 (favorable. Or unfavorable?), it made a difference. I have a clear PSMA, Gleason 8, and will do radiation. So I don't’ think it makes any difference to me?
