Tymlos for life? My endo is telling me this

Posted by zygote @zygote, Aug 23 8:10am

I saw my endo this week and he said he wants me on Tymlos for the rest of my life. I'm 76 and am only starting my 3 month. Has anyone else been told this?
When I started tymlos my heart was POUNDING . Suggestions that I start low and move up in my dose helped enormously... I only had pressure on my heart and now starting the 3rd month the pressure is finally gone.

Weren't you the one who told me that Tymlos CAN now be taken for more than 2 years? If you weren't maybe it was @gently?gently One of you did and I checked it out and posted the link. And it can be prescribed for more than 2 years. However, one is supposed to be then taking the Reclast when they complete the Tymlos for 3 years (the protocol now) and then you are done. Unless there is another reason your doctor finds to continue.
This was the article I believe I quoted from after one of you - I thought told me the restriction was lifted: https://www.ccjm.org/content/ccjom/88/9/489.full.pdf

@loriesco, hey, don't blame me.
It was probably me, but I would have sent https://www.biospace.com/radius-announces-update-on-tymlos-abaloparatide-label
Protocol is rigid--rules that must be followed. Various osteoporosis associations offer guidelines as do the manufacturers pamplets. Doctors have license to vary from guidelines.
I may have even sent the link you cite at another time, but it is about Forteo, you see.

you can order 90 days and take it with you. It is small. You can have it shipped to where you will be or pick it up at a pharmacy where you will be traveling to.

Evenity has to be administered in my docs office…unlike Tymlos which I injected myself. So my option is to wait until we get back from vacay.

The better bone expert needs to write better (newer) articles. One is 2018 and the other is 2020. The information is now old and some of it is outmoded. Here is something from 2021 for prescribers. This is more helpful (and accurate). You can call the manufacturer to see if there is further updates. But I think this is current: https://www.ccjm.org/content/ccjom/88/9/489.full.pdf

interesting article. It does say that continuing forteo may be considered to be used life long for those with COPD/fx. Thanks for your research!

It was someone's endocrinologist. And I'm sure he is correct, but I am also scheduling with him, thanks to this name protected, very valuable Mayo connect member. Rest assured Windyshores it wasn't you. And I will testify on your behalf if it comes to a court of recrimination.

I'm really confused by "Tymlos for life." I take it, prescribed by a rheumatologist who told me one can only use it for two years. Have the guidelines changed???

I’m not blaming you - just asking if there was a contradiction because I thought I heard it from you. And sorry I sent the wrong link to the incorrect reference!!

Seriously, I don't mind just don't blame windyshores. She's innocent.
I'm not sensitive, and enjoy I little dust ups. I'd forgive you, if an apology were warranted.
@bayhorse, the black box for cancer risk has been lifted for Tymlos. Guidelines remain. They are stubbornly slow to change. Some docs are prescribing for three years. Eventually, you'll be able to return to Tymlos again, but would you want to. Your two must be over. Did you suffer nausea all the way through. Did you use the under the tongue zofran.
What's next for you.

@gently lol

@bayhorse Tymlos is still for two years. Forteo can be taken for longer.

Windyshores: Any idea why Forteo can be taken longer? And am wondering how much longer. I thought it was essentially the same drug as Tymlos. Also, can one go on Forteo after finishing the two years of Tymlos?

@bayhorse Forteo came out much earlier. The company did studies on extending therapy. Radius has not done that for Tymlos. There is nothing inherently different in Forteo versus Tymlos in this regard: it is just what has been studied. Studies take time and money and people and I read that it was unlikelu Radius will commit to those.

The issue, in my experience, is that the PTH analogs stop working at some point. This can perhaps be ascertained by bone marker testing though my doctors have different opinions on that. I was told most of my bone growth happened before 18 months when I finally had a DEXA.

Extending treatment may help some who are still responding. I don't know if anyone is. Your doctor could tell you. Doing Forteo after Tymlos might be possible but I haven't heard of anyone who has done that as yet perhaps because of the issue of effectiveness petering out. Maybe someone will chime in who has done this or knows more about it.
