May I ask, how (or who) diagnosed "small fiber neuropathy"? It seems very specific for you to be getting no help with it now.
The itching could be caused by whatever medications you are now taking. That's a common allergic reaction.

Mine was dxed at Cleveland clinic. History. Physical test and ski biopsies. Dx small nerve dinner sensory neuropathy. Cause not known. Pain fluctuates but need nor o 20; 2-3 times per day. Any advice?
