Gleason7(3+4) - treatment options recommendation

Got recently diagnosed with Gleason group 2, 7(3+4). Was in state of shock to know about the cancer.
I’m 56 year old and fortunately I’m with Mayo care since last decade.
Recommendation for me is to have prostatectomy as radiation therapy has long term implications. Took outside opinion also and same recommendation. But not sure how to deal post procedure with urge to urinate situation currently there.
Biggest thing is I’m hoping there is no recurrence occurring after this. Any suggestion/recommendation?

Great info but what does NS RALP stand for? I have not seen that acronym, or if I have I am not connecting. I assume NS stands for non-surgical..

@pmclarksr -- with surgery, there's a variety of options so when someone says they had surgery it's important to know what kind. As already mentioned NS RALP is Nerve Sparing Robotic Assisted Laparoscopic Proctectomy. Mine was bilateral, so they could preserve the 2 nerve bundles, one of each side, as opposed to unilater where they can only preserve one side nerve bundle, or NNS (Non Nerve Sparing) where they take both nerve bundles. One of the questions I had before surgery was whether I was a candidate for bilateral NS RALP as that would give me the best option for being continent and eventually having sexual function. Sometimes ahead of the surgery they already know they'll have to take more. Other times they may decide they need to take more tissue once they examine the tissue during surgery. Similarly, when a surgery goes well without complications the truama to tissue is minimized, while other times the surgery report will state additional measures were needed.

If you're talking to the surgeon, don't be shy to ask how many of these surgeries he's done and what his incidence of incontinence and ED have been, especially for cases like yours. I've read that most good surgeons will track their numbers and be willing to share them. I asked these questions as I wanted a surgeon who had a lot of experience with this procedure and a high success rate. This is just more information to help you select the best treatment option for you. Best wishes.

I agree. I remember a long time ago using saw palmetto to keep my PSA down. My urologist warned me that it produces a false negative PSA result . Don’t know the biochemical reason why but perhaps mushrooms do the same thing?
A friend of mine took a pharmaceutical dose of green tea extract for his leukemia rather than chemo. His doctor at Leahy Clinic in Boston actually encouraged him on this. IT WORKED! His blood was good for years.
Now Green Tea is also touted as lowering PSA and fighting prostate cancer (and others). But the guy GOT prostate cancer and had surgery. So be careful of JUST looking at PSA even though it’s the only real marker we have right now aside from certain genomic tests.
BTW, my friend beat leukemia, beat prostate cancer and then fell off a ladder trying to clean his gutter on the second story of his home. He is still confined to a wheelchair to this very day….sometimes you just can’t win…

Not that I knew at the time .
But tomatoes have been touted as a cancer preventative . For years and years , I have eaten bushels of tomatoes -- Guess what : I have prostate cancer , GO FIGURE .

Yeah…and ginger (took it), turmeric (took it), DIM (took it, Vitamin E (took it), frequent ejaculation (overdid it!)….I think I CAUSED the cancer😂

The trick with tomatoes is you must cook them, eating them uncooked doesn’t work. Very frustrating since I have them uncooked every day almost

Have you read about ablation treatment for prostate cancer? If not, there is a thread on this blog called "Ablation, has anyone heard of this type of treatment?"

While it seems to be only for those guys with the cancer contained within the prostate, it might be worth looking through the comments on that thread.

I cook them 95 % of the time . I know cooked is best.

Like you I took them all . Green tea , garlic etc --- All gimmics .

one item to look at if you go operation, your age is on your side . Your young and heal great ! I was young too when diagnosed .You also get full picture of Prostate via biopsy after operation - valuable. This is on your side . The 3+4 is MUCH better than 4+4 . Gob Bless you and yours Sir .
