Dental work while on Reclast

Posted by hlp123 @hlp123, Oct 4, 2022

I had an infusion of Reclast in March 2022 as a transition drug to stop taking Prolia. I had a tooth break off at the gum last week and saw an oral surgeon yesterday and he said he needs to remove the root of the tooth before it becomes infected. Has anyone had invasive dental work, such as this while on Reclast ?
Please let me know about your experience.
Thank you,

Thank you !
When I start using adhesive, I will definitely follow your suggestion and use three dots of adhesive. I appreciate your help. I am so SOB that I hesitate using anything in my mouth.
thank you,
Holly

It is understandable that you would be cautious about any products you put in your mouth, Holly. I hope you are able to find something that helps as you adjust to the dentures.

It's always good to read your posts, so keep in touch!

Dear Lori, As soon as I read your reply, I relax and feel that I know what to do. You always give me a plan to move forward with and having a plan makes my anxiety go away. So I called the dentist and they told me to call the oral surgeon. I got an appointment with the oral surgeon, but as you said, they usually have a full schedule so he can't see me until next week. I should be fine until then, because it does not hurt.
I have not tried any adhesive yet. I admit that I am hesitant to try adhesive. I think that it may be because I have severe COPD and the thought of having something "stuck" in my mouth makes me uneasy. I am sure that this will pass and as I adapt to the denture I will be ok with adhesive. I'm just really taking things slow. Thank you, again, for being there for me. You always seem to know exactly what I need.

Good morning, Holly. ☺️ It helps to have a plan, doesn’t it? Instant stress reducer.
It’s great you’re able to see the oral surgeon next week. That should be an easy peasy appointment to coax that little sliver out. Your mouth will feel better and your tongue won’t be drawn to that spot, exploring all the time.

Teresa has a great suggestion about using the adhesive with dotting the denture in 3 places. It doesn’t have to be all or nothing. That should be enough to anchor the denture without being fully adhered. I absolutely understand your feeling of claustrophobia if you have COPD and fear that you can’t get the denture out quickly needed. But by using even a small amount of adhesive, you’ll have a much better fit with the denture right now, with less fear of it letting go at inopportune moments. 😉

Stay cool! It’s so oppressively hot that I’m losing my cherub-like demeanor in this stuff. LOL. 🥵

Dear Lori,
This sounds like a sensible plan. I appreciate your empathy with my situation. As you well know by now, I tend to overthink things and then I get anxious. I'm working on recognizing the "what If's" and trying to relax.
You are the Best !

Hi @babse. I’m sure the prospect of possibly losing your teeth as a result of your MS and medication is very concerning and you have a lot of questions. I’m not a dentist but have a strong dental background having been a certified dental assistant for 30+ years.

What is reason for needing dentures? Is this both arches? Has there been significant bone loss around your teeth and you’re experiencing periodontal disease?

I took Reclast 2 and 1/2 years ago (one infustion) for osteoporosis and had an extraction of tooth number 14. It became infected and there was an overgrowth of tissue. The oral surgeon removed the tissue and prescribed antiobotics. Number 15 was already missing and one of the lower teeth on that side was missing. I inquired about an implant of number 14 because it is now hard to chew. The dentist (it is a dental school) said no implant because Reclast has a half life and no more extractions because the last one was slow to heal. My osteoporosis doctor said an implant is ok. I also have cll which might make me immune ompromised so it could slow down healing. My other options are to do nothing, have a partial which is removable or go see another dentist who might do the implant. any ideas of how to proceed?

Hello @justinesmith,

I moved your discussion and combined it with an existing discussion titled, "Dental work while on Reclast" - https://connect.mayoclinic.org/discussion/dental-work-while-on-reclast/.

It is a bit of a lengthy discussion, however, it has many members discussing all things dental procedures while on Reclast or having taken Reclast in the past.

@hlp123 started this discussion awhile back and has had many experiences they may share with you.

@justinesmith - your osteoporosis provider has greenlit an implant but your dental provider will not do it. Howe feasible is it to get another dental opinion? Sometimes that is not always an option. Would it be possible to have an open line of discussion or medical documentation from your osteoporosis provider to share with your dentist?

I also need oral surgery after reclast. I am in West central Wisconsin and go to Mayo Clinic Health System in Eau Claire. It is very hard to get any dental care here if you are on Medicaid. I was on a waiting list a long time. I don't have any molars and my front teeth are all broken to nubs. The plan was to pull everything and go with dentures. I asked about an upcoming reclast infusion. Nobody let me know it would affect my oral care. Now the doctor won't do any extractions. Is it possible to get a second opinion from oral surgeons at Mayo? I can't eat a lot of foods. I have pulmonary hypertension and worry about infections. Thank you

How long ago was the Reclast? Can you discuss with the doctor who did the Reclast what the risks are? Can you have a blood test to see how much of the Reclast is in your system? Can you have root canals and crowns instead?

I had the reclast last May. The oral Surgeon and the endocrinologist were supposed be in contact and I was assured that I could go ahead with the reclast and it wouldn't cause any problems with dental extractions. Then they oral surgeon said no and the plan is to pull teeth one by one as they become infected and cause pain. I just can't believe this is a valid alternative. I am sick about it, in tears just typing this comment.
