What options for poorly differentiated carcinoma, Keytruda failure

Have poorly differentiated carcinoma of unknown primary, Stage 4, diagnosed 06/2022. Initial treatment with FolFOX and then added Keytruda when PD-L1 returned positive. With Keytruda enlarged lymph nodes rapidly resolved. Now recurrent disease despite ongoing Keytruda. Awaiting recommendations for additional therapy. Had extensive staging: CT, PET, 3 lymph node biopsies, EGD, Colonoscopy, EUS, Capsule study, Genetic studies, etc.
Are there others with Stage 4 carcinoma of unknown primary and Keytruda failure?

@cindylb shares a similar story to yours here https://connect.mayoclinic.org/comment/333576/ where Keytruda worked at first and then failed as a maintenance therapy for carcinoma of unknown primary. His cancer was being treated with protocols commonly used for lung cancer. @frankbc may also have experiences to share.
