Poor range of motion after TKR and manipulation: What to do?
I had a TKR 3months ago and had manual manipulation 4 days ago. My ROM was great while on the nerve block but as soon as that wore off my ROM went from 110 to 70 degrees. I do have a lot of pain in the quadricep and the muscle are very tight. I am taking a muscle relaxer with no relief. I am thinking of going for a second opinion. I had a comformis knee replacement and I am going to PT 5 days a week without much success . Does anyone have any recommendations or have had the same problem.

I have had so much trouble since my TKR. PT essentially said they couldn't help, I needed to go home and "heal." My entire leg vibrates. It is maddening. It has been 5 1/2 months and I am so 😞 discouraged. I had a genicular ablation and not seeing much benefit. HELP?
Right Knee History
2018 Road Bike Fall
2019 April Root tear to meniscus. Surgery to repair root tear to meniscus. failed
2021 Partial Knee Replacement EmergOrtho in Durham NC. Surgeon retired end of 2021. PKR failed.
2022 TKR Carolina Ortho, New Bern NC Lost flexion due to scar tissue Flexion stuck @ 72 degrees
End of 2022 surgeon put me under to perform Scar Tissue removal surgery (open Knee up), he attempted MUA - it worked - he was able to get flexion to 116 degrees - he did not do surgery. I attended PT every week for many many weeks. Flexion decreased again due to Scar Tissue.
2023 surgeon perform open knee surgery to remove scar tissue to regain flexion. Attended PT continuously, flexion of 113 degrees was accomplished. . Feb 2024 pain presented behind right knee. Currently in PT, recently presented to Duke Ortho in NC - CT Scheduled for 7/12/24 - 2 scenarios - bone spurs or fragment of cement left behind from TKR or scar tissue impinging popetiel tendon. Experiencing extreme pain daily with aching, sometimes numbness. my gut feeling is the scar tissue scenario affecting popetil tendon. If that the case, Dr Cline at Duke will perform Ultrasound Guided Percutaneous Tenotomy. Has anyone had this procedure done to their knee???
I admire your patience and perseverance. I am on your same road, and praying I can stay the course as you did. Had trauma from the MUA and am 6 weeks in recovery from that. Bleeding and quadriceps hematoma resulted, excruciating tenderness and Pain, no weight on operated leg for at least 3. 1/2 weeks, now about 80% weight bearing. Next step mentioned is the surgery to remove scar tissue. Leg still swollen, so I hope it can wait to let it heal more from the MUA. It seems that once you’re on the difficult recovery road, you dont get off for years! Choices are difficult.
I think I probably should have had an MUA. Got a notice from insurance that the genicular ablation is considered "experimental ," something that was NOT conveyed to me. But, pain management is covering.
I have a friend who had something similar to your situation. Took a very long time for recovery and additional surgery. I wouldn't have had it if it wasn't the only option. Some days are better than others. I'm glad I found support. ☺️ Please feel free to reach out if you need to.
Reading the article doesn't really mention the big secret the ortho Dr.s do not want discussed- wrong sizing possibility.
Have to dig deep to read about this issue causing problem.
Hi all I am where everyone is had tkr was too big year later had revision went amazing felt awesome only was able to get to 105 flexion but workable……never was totally out of pain I just thought going to take time and dr told me would always have some pain because of being revision …. I am at lab now gettin Ltt test checking for my body having issues with metals used…. I don’t know I was exercising 5 days a week had to slow down was affecting my other knee which is bone on bone and. Am not getting done …. So I have to not be so hard on it …. I recently started organic caster oil it is said to get into the deep tissue break down scar tissue so heck I will try… I wear pad all the time I will let you know in few months if it works…..my knee is tight pain in back of knee ….so I will see what test say go from there …..good luck to all …. Hope you find relief
7 weeks post-op, I can get to 120s & 0 now at therapy, after 5 mg oxycodone 1 hour before and 2 therapists working on me, 3x/wk. I do my therapy at home as best as I can too.
But every day is like starting over. Terribly tight, slow to warm up. Still taking Motrin and Tylenol to move. Very tight to bend. Anterior pain that’s moderate with walking. I feel I am walking crooked and now my back is starting to be sore. I am to do the other knee but am reluctant until I see some light at the end of this first one.
Am I going through normal tightness or is this an issue with size?
I so appreciate this forum. Tried something like this on Facebook and it was removed. Scary stories not allowed I guess?
We have to be informed and learn as much as possible when talking about a body part that we need for ADL!

Hi Cathy - I am 5 1/2 weeks since my tkr. I am experiencing the same problem you are. A change that seems to be working for me is that I now use a heating pad to improve blood circulation and “soften” my scar tissue. After about 20 minutes of heat on my leg and knee, I begin bending my knee. I modified how I stretch my leg. I sit in my chair with my foot on the floor. I then slide my butt forward to really bend my knee. I push on my knee to my pain limit. I then start my phone’s timer. I try my best to distract myself the pain as best I can. When I reach my limit of pain tolerance, I stop the timer on my phone. Very uncomfortable, but the constant pressure on my knee seems to have helped my range of motion. Before I started doing this stretching technique, I couldn’t bend my knee 90 degrees. I am now up to 105 degrees. Bending my knee is not fun. My goal, which I keep track of, is to “endure “ 10 minutes per day at the highest pain level I can tolerate. Some days, I can only tolerate 20 seconds at a time. Other days I can tolerate more. I just keep going until I reach 10 minutes according to my phone stopwatch. One major key to success is DISTRACTION to keep my focus off the pain.
