Prostate cancer AND bladder cancer - any others with both?

Posted by mselby1847 @mselby1847, Apr 7, 2023

My husband has high grade bladder cancer in a diverticulum and also prostate cancer with Gleason score of 4+3 plus. TURBT surgery 3/14/23. Urologist has recommended radical Cystectomy and removal of prostate, etc with a conduit and bag. He’s 59. Our daughter is a PA and all of her research and mine agrees with this approach. He is scheduled at Mayo for 4/24/23 for the surgery so I’m assuming that the surgeons agree with this radical approach as well because of the dual cancers. Does this sound reasonable? It just seems so fast to go from a CT scan in Feb to radical surgery two months later. My husband is ok with all of this and wants the cancer gone so he can get on with his life. He doesn’t want to keep doing cystoscopies and prostate biopsies every three months and radiation etc. just one and done. And he doesn’t want to have recurrence scares.

Sounds like the same protocol as my husband. He’s only had one cystoscopy that was clear in the last year. He still gets them every three months. Well done.

UPDATE since 4/23!! Husband went in for tests prior to surgery and they discovered he had a PE in his lower right lobe so hospitalized with heparin and cancelled surgery. Put him on blood thinners and did six weeks BCG treatments. Every 3 mo did cystoscopy and scraped and burned small spots if possible cancer in bladder. Did a prostate MRI in late Oct and discovered his prostate was FULL of cancer (three large tumors and invaded to nerve bundles) so it had become urgent to do surgery (non nerve sparing). He has decided against bladder removal and urostomy bag until bladder cancer returns full force (doctor agrees).

That was quite a turnaround @mselby1847 but it has saved his bladder for now. That PE risk with bladder cancer is very real. My husband was on blood thinners until his bladder was removed and all cancer gone. Prostate was removed with that surgery as well and he did have a low grade cancer present there. I hope your husband's cancer remains under control for many years. Did he need to have chemo due to the invasiveness of his prostate cancer? My husband is now on every 6 month CT scans watching for metastatic disease. He has had lots of issues and complications from neobladder surgery but is still glad he chose that over urostomy.

The prostatectomy took the nerve bundles and 25 lymph nodes as well as the prostate, of course. No cancer cells in any of the lymph nodes so they determined no metastasis and no chemo or radiation. We were glad for that as it gives a better outcome should he eventually need his bladder removed. He will be on blood thinners until he has zero chance of bladder cancer (which may be never). He HAS to work until he is 65 in order to keep his insurance as we can’t afford self insurance. His work insurance still has a $10k annual out of pocket and costs $400 a month for just him. So he has four more years to work and doesn’t want to deal with a urostomy bag as a truck driver if he can avoid it. So we go every 4 mo to check.

Yes, my bladder cancer was discovered during robotic prostate removal. Tumors kept coming back but were always Grade 0. Six BCG treatments has stopped it so far but prostate cancer is returning and urologist fears radiation may cause bladder cells to become more aggressive. Radiologist says it’s not something we have much choice about so going for MRI and PET this week to see what’s what. Nervous!!
But invasive bladder cancer will kill you a lot faster than prostate - I’ve seen it. You can live for years with PCa and get treatment with hormones, chemo, SBRT for metastases, etc. but bladder cancer is much more difficult to treat once it spreads so perhaps focus on that and see what your doctors think. Good Luck to us ALL!!

I have bladder-c for 6 yrs. Now c is suspected in prostate, so HoLEP is scheduled for 091224. What can I expect for effects after this procedure?

Prostectomy 11 yrs ago
Full nerve sparing non muscle invasive at ucla
7 yrs ago had nmibc
Bcg did not work
Some invasion into urethra
Had bladder removed and internal pouch built
I live an active lifestyle so external pouch was not an option
NO issues in 7 years
