My original pulmonologist thought it was the result of long-term asthma, extensive exposure to irritants (paper dust, asbestos and soil particles) and repeated bouts of bronchitis and pneumonia, going back to childhood. In addition, I meet the criteria for "Lady Windemere Syndrome" - older female, slight frame and lifelong low body weight.

Related to all you wrote above....
Certain events that we have lived through in the last couple of decades brought me to the conclusion that things happen and more than likely it is more than one thing (a culmination of things and events) that caused the problem, sad event or health problem.
I think one could add STRESS to one of the reasons we find ourselves being diagnosed with BE. Stress causes harm to the immune system, we might go into depression and as well either eat wrong or just not eat. My two cents worth because of my looking back to answer my question to myself..."How did this happen, what caused this BE."
I can repeat a a good couple of things you mentioned along with the stress factor I was in from childhood and in adulthood. Forward we go with the knowledge and understandings we now have!
