I have asked this question to both my RP surgeon and GP - neither has had a satisfactory answer. I had laparoscopic RP in late August 2023. I had no other treatments (radiation/hormone therapy, etc. - as cancer was contained). Had urinary tract infection during preceding month (probably when catheter was in). Was resolved with antibiotics. However, I have had penile sensitivity/pain pretty much since surgery. It is not 10/10 pain - but it is an awareness. It is in no way "arousing". When I squeeze (anywhere from tip to base there is pain. Sometimes just pressing on the tip with tighter clothing/underwear/bike shorts it elicits pain. I know the urethra is cut during surgery and perhaps this has something to do with it. I have been doing aggressive kegel exercises since surgery - usually 3 x/day. I have also been doing red light therapy on perineum (as it has also been sensitive/painful to direct pressure). Sometimes I think I have a UTI again because of the pain - I have had myself checked twice for it - but was clear. I have continence about 90% under control. I am using sildenafil and taladafil (?) as part of rehab program for ED. Blood flow to area also causes sensation of pressure and a sense of moderate pain. All this to say, I do not have an answer - but I can identify with your symptoms. I plan on re-broaching the subject with my GP next month.

I’m reading your post and identifying. I had RP and no other treatments and will be 3 years in October 2024. A lot of testing through infectious disease specialist turned up all negative. 2 cystoscopies about a year apart ( very painful due to my condition) reported some mild stricture in the urethra however uroflow check is adequate to empty my bladder. One urologist thought maybe lichen schlerosis and another said no just wait it out. I definitely did not fit the bill for LS. And will not agree to biopsies of the urethra 😫.
I’m not doing any more invasive testing and am improving ever so gradually. Sometimes a step forward and then back. It’s almost 3 years and I’m no where close to never giving it any thought like I was pre surgery but psa is still undetectable and I’m living with it.
