@kellybelter - what is his physical history? With work and activity.
Did depression set in before pain or after?
Was there one day when it all started? As one event of pain, can lead to limiting movement, which then causes the wider spread pain and less movment which then turns into depression which worsens all over symptoms.

He owns a tube bending business. There was a lot of manual labor, so I am sure that plays a role. He is very depressed and stressed and I know that makes things worse.
