My brother has had the same problem after surgery, very nauseous and disoriented for hours after waking up. It's not uncommon. I don't know of there is anything that can be done, other than to alert the anesthesiologist and possible get some anti-nausea medicine after.

Staying two nights due to pain for a TKR is unusual. We all react differently.
