I had a prostate cancer surgery and removed the grand 3 years ago when the greasiness score is 7. Since then I found on my PCSA is increased double or even 5 time more.( from 0.008 to 0.033, then 0.035 , 0.074 to now 0.37).

This seems to be a bad sign , very likely my prostate cancer comes back, I would like to have some advice about what I should do for treatment. I am 77 now now among few different treatments such as chemotherapy, radiation, hormone etc. but don’t know which is the better treatment for my age and health situation. I have atrial fibrillation but no other health problems, and keep active, and regular life style, such as swimming and walking every day. Your experience is highly appreciated

My PSA was .19 after surgery in 2022 at age 72.
Salvage Treatment of radiation together with short term ADT Feb - June 2023 at age 73.
See SPPORT trial.
Currently 1 year post salvage tx and PSA just reported < .02
If PSA continues to rise and if PSMA PET scan is negative for spread, then pelvic radiation might be appropriate.
If a lesion is identified, then radiation of that lesion could be indicated.
