Need Advice - What drug to Follow Up Evenity
I finished my 12th injection of Evenity on March 28th. It’s the only medication I’ve taken. Doctor recommended Prolia. I told her I would do ONE only. Then Reclast, then off drugs. My goal is to be able to ride horses for the rest of my life. I was diagnosed after I broke my shoulder. I’m 66. I suspect I have good bone quality. Started HRT.
Is that a good strategy?
Can you go from Evenity to Forteo?

Hello…I was denied Tymlos when my doc first recommended it. Insurance wanted me to begin a regimen of Fosamax but because of my high loss -4.7 my doc appealed the decision and it was overturned. I finish 2 years in July.
Great improvement on Evinity!!!
I’m just through with evenity and two docs told me I could wait a month or two at the most…& I’m trying to find out if during that interim that I can get my tooth pulled and other dental work needed( the hard part) I was advised to take Reclast because if Prolia doesn’t work you go to Reclast anyway so it avoids jumping around…
Anyone stuck in doing dental work?…I hope I can work out the time!
Hi @prehorselover - Sorry for the delay.
It looks like you had really good results on Evenity, especially in the hip area. Based on the main Evenity trial that was followed by alendronate, I would start alendronate (Fosamax) ASAP. Get another DXA scan in two years and maybe you can take a break from drugs for a couple of years.
I would not transition to Prolia as you really don't need it in my opinion.

It is well published that your first line of therapy should have been an anabolic. Great that your Dr challenged the bias insurance comp!

My PCP prescribed Fosamax, but I didn’t trust her opinion. I don’t remember if I filled the prescription, but I didn’t take a single pill. Then, I got a second opinion from my orthopedic surgeon, who recommended Evenity.

My goal is to increase my bone density enough that if I fell off a horse I wouldn’t fracture. I want to start riding my young horse in 18 months.
Is that possible with HRT, Fosamax, and jumping rope several times a week?
Hi @prehorselover - It's pretty difficult to put a probability of a potential break when a horse falls on you. I assume it's pretty high regardless of your T-scores. 🙂
If I were you, I would try the alendronate + HRT for 18 months and get another DXA scan to see where you're at. I'm not sure what target you should be shooting for specifically, but if you feel it's still to low, then you could do another round of Evenity. That should really push you over the top.

Hopefully, I’ll never be in a situation in which a horse falls on me! I’ve fallen off a horse many times in my life and I’ve never been seriously hurt. It happens. I didn’t think twice about it until I was diagnosed with osteoporosis. I don’t want to worry that I might break my hip if I come off.
