I was concerned about that, too, but they do stay down. Evidently the fiberfill has enough weight. You can adjust the size by adding or subtracting fill and they still stay down. I've worn mine for over a month and have been very pleased. They are cool, soft and fit into any bra you buy. No need to buy the prosthesis bras. And it is cool to the skin! And I live in Arizona!
