Gravity-
This is a site everyone should visit and study.
"Shared decision making" seems to be the core idea.
We don't live near a Mayo Clinic, but we do live close to two major university based medical systems (Duke and UNC), so we have many doctors to choose from, and we have used that option when we felt a doctor wasn't listening. My wife has had three major surgeries in the last three years and finding the right doctors who would really consider the reasons for our choices was crucial to getting very good care that resolved the problems.
Whenever I visit a doctor I bring a written note stating my concerns and up-to-date information, one copy for the doctor and one for me.
The Patient Revolution is a site everyone should visit as @edsutton recommended. Here's a great YouTube video with the story on how it started.

The Patient Revolution is alive and well. It is also known as the Hospital at Home, Mayo's Advanced Home Care, and other terms. Covid pushed it into the limelight, and it has not slowed down.
Many people need to ask if you will be ready for its impact.
I ask Microsoft CoPilot.
"Give me a list of the top review articles on HOSPITAL AT HOME, ADVANCE HOME CARE, and research from the last ten years."
Here are some relevant articles that discuss how Hospital at Home and Advanced Home Care are revolutionizing patient care:
Hospital at Home:
Hospital at Home is an innovative care model that provides safe, high-quality, hospital-level care to older adults in the comfort of their own homes. Developed by the Johns Hopkins Schools of Medicine and Public Health, it has been tested at nationwide medical centers.
Key features:
Quality Acute Care at Home: Patients receive acute-level care without being admitted to a traditional hospital.
Cost Reduction: This model lowers costs by nearly one-third compared to hospital-based care.
Patient Experience: It is highly rated by patients and caregivers alike.

Hospital-at-Home:
The Hospital-at-Home model enables some patients who need acute-level care to receive treatment in their homes rather than in a hospital. It has been shown to:
Reduce Costs: By providing care at home, costs are lowered.
Improve Outcomes: Patients experience better outcomes.
Enhance Patient Experience: The model prioritizes patient comfort and safety.
In November 2020, the Centers for Medicare & Medicaid Services launched the Acute Hospital Care At Home program.
