I don’t know if you’ll have any luck with this, but you can try to contact the PT yourself and cc their superior, and ask that he/she amend their note, and remove that as a formal diagnosis. I’m a mental health professional myself, and I wouldn’t diagnose somebody based on one comment they made. It’s a little gratuitous. They didn’t discuss it with you further or ask you if you felt you could use help with your anxiety. But try not to let it bother you so much. Not everybody reads a person‘s entire chart; and you can always tell your new PT that you feel you have been erroneously labeled. Many charts I’ve seen allow the patient to ask that a particular diagnosis be removed and ask the patient to explain why. Good luck!
