Bone Coach - How do you know it works?

I have been using Tymlos for 7 months. I just had a scan, although the bone clinic here did not do one before I started and usually does not do on before you have been on it for year. However, I have heard that the largest gains are made in the first months of use. The scan showed slight improvements in my wrist and lumbar spine over the last scan two years ago, but my hip was slightly worse. It could have gotten worse still after the previous scan and improved after all but we have no way of knowing. Two of my liver enzymes are elevated; it seems that is not unusual for ATP but I haven't found anything mentioning AST. Still, Tymlos seems to be a little more effective than Forteo, and possibly safer than Evenity, which substantially increases the risk of major cardiac events in some people. It does preserve old bone as well as help build new bone, but not in the same way that bisphosphonates do.

What kind of scans are you getting done that checks the density or the fragility of your wrist?

Fact check here - there is no evidence that Evenity "substantially" increases cvd risks. There was one study where the Evenity side of the trial did very slightly worse than the biophosphnate on the other side in number of cvd events. In another trial Evenity was the same as placebo for cvd incidents. So I would say there is a slight possibility of increased risk with Evenity. Some "top osteo docs" have stated in interviews etc that they suspect there is no increased risk with Evenity for cvd.

Nothing suggestive of cvd incidents has shown up in osteoporosis forums that I know of. The only safety factor that I know of that weighs against Evenity is just that it hasn't been in use as long. Sometimes problems don't show up for years. Full disclosure, I'm getting my fifth Evenity shot today.

@imay how long will you be on tymlos and what is your long term plan for the next 20 years after tymlos? I have read that after tymlos and/or forteo it is important to go on a bisphosphonate and fosamx is not strong enough meaning the only choice is ReClast infusion. I’m unsure what’s next after the infusion but I also read if you don’t do that you lose the gain and even go below what was gained from the tymlos or forteo. Bisphosphonate is exactly what I chose not to take 2 years ago when my Tscores indicated osteoporosis. I just turned 66 lastbJuly my hip score was -2.6 and spine -3.3 tymlos forteo or evenity was prescribed by my endo to reduce fracture risk but the side effects seem worse than the disease. How did you decide to turn to Tymlos and again what’s next. Thank you for your input.
