Admission Process

For Patients: Contact your doctor and express your interest in attending the PRC. Your doctor will then send the PRC team a referral. The PRC team will reach out to you once they receive the referral sent from your doctor. If you do not receive a call from the PRC after your doctor sends the referral, contact your doctor and ask for them to reach out to the PRC.

For Referring Practitioners: To refer a Patient to PRC (Pain Rehab Center) in Florida have the patient’s medical provider fax the following information to the Mayo Clinic Referring Physician’s Office (RPO):

  • Referral for 3-week Pain Rehab Program
  • Current general medical examination documentation (within 1 year)
  • Current list of medications
  • Medical records pertinent to chronic condition and imaging
