The concussion team at Mayo Clinic is excited to continue advancing sport safety with the Coyotes Amateur Hockey Association. Our concussion program provides evidence-based remove-from-play and return-to-play concussion protocols designed to protect athletes at all levels of play.
Mayo Clinic's Concussion Check remove-from-play program is taught to non-medical team volunteers to assist in determining when an athlete should be removed from sport activity following a suspected concussion. It includes a three-step protocol using the King-Devick Test in association with Mayo Clinic. Concussion Check safety officer training and King-Devick testing are coordinated at the league level.
Mayo Clinic’s preseason return-to-play assessment includes the completion of our concussion history form and computerized cognitive test (Cognigram). This information assists concussion specialist in evaluating a potentially concussed athlete and helps in determining recovery.
Complete the Concussion History Form. The athlete and/or guardian are required to complete the online history form prior to selecting an appointment time.
Select an appointment day and time to complete your computerized cognitive test on the Mayo Clinic Scottsdale Campus.
Sign and date the attached Mayo Clinic Authorizations and Service Terms form. IMPORTANT: You will need to bring the signed form to your appointment. Minors arriving for testing without a form signed by their parent/guardian will not be able to complete testing.
Attend your concussion assessment appointment. Please arrive 10 minutes prior to your assigned time. Download parking and building directions.
Should an athlete be removed from activity due to a suspected concussion, the athlete/guardian should seek healthcare with a concussion specialist and notify the league safety coordinator. The athlete and guardian are asked to contact the Mayo Clinic concussion team within 72 hours of the injury. A team member will assist the athlete/guardian in completing post-injury assessments. Please note, this program does not include post-injury concussion healthcare by Mayo Clinic. Should the athlete/guardian want to be seen at Mayo Clinic and their insurance is compatible, all post injury assessments will be completed in Mayo Clinic’s Return-to-Play (Acute) Concussion Clinic on the Scottsdale campus.
Should the athlete/guardian want to be seen by another medical facility other than Mayo Clinic or if their insurance is not compatible, a Mayo Clinic team member will assist the athlete/guardian in obtaining all preseason assessments and coordinate post-injury testing with the doctor providing care.
