When Biopsy and immunology reports don't match IGA

I have a biopsy from 2024 stating I have IGA in my kidneys, I have an immunology study 2025 stating I have no immunological issues. What do you do when the two don't match. Kidneys took a hit in 2023 when I got food poisoning in Mexico and after 2 days of dehydration, my son showed up with gallons of electrolyte. The doctor at the time told him to force the fluids down me at the hotel. This landed me in a Seattle hospital for 5 days. Kidney function went from 70 to 35. Back home, GFR is steadily declining and now at 18. They have me on Mycophenolate and prednisone, but really with no conclusive evidence that the immune system is still acting up. Any hints out there? I have requested Mayo rerun the tests as I am beginning to suspect the laboratories in this area.

Hi @u4gmlglm you have 3 discussions underway all pertaining to kidney conditions (biopsies, studies, tests, treatments). You’ve had a lot going on (dehydration, food poisoning, hospitalization). It is no wonder that you’ve been experiencing a declining eGFR. It has you wondering if the condition is acute or chronic (CKD) and if there is an immunology component to your situation or not. You said that you are taking Mycophenolate and Prednisone. Do you know why you are taking these? Who prescribed them? Mycophenolate is an immune suppressant. Prednisone is a steroid commonly used to treat inflammation. You mention being in Mexico and being in a hotel in Seattle where a doctor told your son to “force gallons of electrolyte fluids down you”. None of this sounds like a good way to care for yourself.
IMHO you need to get established with a provider or team of specialists with you being part of the team. Make sure everyone has copies of your medical history (tests, diagnoses, medications & treatments ….) so that you know you’re all on the same page so to speak. Then set goals and priorities to move forward with a treatment plan that will best meet your needs. I wouldn’t keep moving around from Dr. to Dr., clinic & hospital to another, and med to med hoping for the best.
