TURP for incomplete bladder emptying

Posted by kfrancis @kfrancis, Sep 13 6:51pm

I have chronic incomplete bladder emptying despite using Tamsulosin. Urodynamic testing indicates that my bladder contracts strongly, and my prostate is normal size but is shaped in a way that obstructs the urethra. I am considering a traditional TURP. For anyone with similar retention and a functioning bladder: how much did your post-void residual improve, how long did improvement take, and were you able to stop tamsulosin? Overall, are you satisfied with the outcome and would you recommend the procedure?

Hello @kfrancis,

I'd like to invite @cowboy10, @stew80, @dwwilliams164, @divemaster, and @brucekn to this discussion to share how their TURP procedures helped, or didn't, with bladder retention. Some shared their experiences in the following discussion:

@kfrancis, it sounds like you have tried a few other alternatives with no luck on emptying your bladder. Is TURP what was suggested as the next step for you?

Thanks Justin.
As previously written, my first TURP was done with regard for preserving the nerves for sexual function. However, urination-wise, it didn’t work and I ended up always wanting to pee. A second TURP, done very aggressively to make sure the urination function was fixed, destroyed many of the nerves that resulted in ED. And urine retention was still present after that second TURP.

I very soon got another urologist, and after sone time recommended a UroLift procedure, which once again addresses a constriction in the prostate. But after that there was still urine retention, somewhat less than before but over 2 years became bad again.

It was after that when my urologist started looking at my prostate and bladder with ultra-sound and an MRI. He was amazed to discover that the prostate wasn’t restricted internally, but had entered into the bladder, forming a ridge around the neck of the bladder, So, urine would pool inside of this ridge and was unable to get out. A subsequent aquablation procedure got the job done, I have close to zero urine retention and PSA levels (I just did one last week) is 2.0,

My advice, don’t rush into a TURP procedure, it may fix some but nit all issues with urine retention.

I recently was treated for a uti and currently taking Flomax for bph. The issue was an allergic reaction to the drug nitrofurantoin. I finished the drug, 7 days some side affects while taking but manageable, but shortly after discontinuing the treatment I had coughing, sire chest, night sweats, headache etc. has anyone had a similar result from this drug, and how long did it last.

I did Holep procedure as I also had a large median lobe protruding into the bladder plus a 70 gram prostate. After surgery, I voided completely, no burning, no incontinence, minor blood for 4 days...expect retrograde ejaculation but no issues with ED. Highly recommend Dr. Xu at Northwestern in Chicago.
