Spine Osteoporosis
Do you have to take a break after taking Fosomax for Osteoporosis?
I took it for 10 years and I was not told otherwise.

From the FLEX trial, women who stopped alendronate after five years, instead of continuing to ten, experienced more clinical vertebral fractures, 5.3% versus 2.4%, though no difference in hip or other nonvertebral fractures.
Since women are most at risk from vertebral fractures, your extended us may have been good for your spine.
But to your question a break after ten years is well advised. The length of the pause might depend upon which medication you would transition to.
With Evenity a brief "wash out" period of a couple of weeks or a month is sometimes advised because the bisphosphonates stay attached to your bones and can blunt the effect of the follow-up medication. For the same reason a break of 2 to four months is advised with Forteo and Tymlos.
The wash out period is advised not because starting right away would be harmful, but because you'd get stronger response from the follow-up medication if you let the bisphosphonate continue to work. When the bisphosphonates work, they remove some of the bisphosphonate from the bone.
I don't think a provider will want to resume the bisphosphonate or other antiresorptive because this class of drug leaves you with a less vascular bone.
Pathologists tell us that the alendronate can stay in or on the bone for more than ten years after we stop taking it. So you still have some protection from the ten year use.
