PSA still .04 after nine months on doublet Orgovyx and Nubeqa.

This is great. As long as it doesn’t start rising and doubling at some point in a short time, then he he’s doing just fine. In my case, I had surgery and it took 3 1/2 years before my PSA started rising,

After 16 years of prostate cancer and four reoccurrences I am on the same drug combination and I’ve been undetectable for 34 months. Those drugs work real well.

PSA of 0.04 is amazing! The doublet therapy is clearly working for your husband. 💪

The regular test can't even detect PSA below 0.1, so they must be using the ultrasensitive one to be able to see it at all.

For reference, the LIBERTAS trial underway for ADT "holidays" with a related doublet therapy (ADT + Erleada) doesn't unpause ADT until PSA climbs back to 10.0 (!!)

I am familiar with that trial, I think I heard about it from you. I didn’t know they were waiting for PSA 10 to resume treatment! That sounds kind of scary but such a finding might completely change the SOC with ADT treatments. Amazing. Our doc suggested we look at the Embark trial which we did. He has mentioned intermittent treatment in the past so I know he is considering it in our case. Do you know when the Libertas trial is going to conclude?

@ucla2025 LIBERTAS is planned to finish in late 2027, with preliminary results coming in a few months.

Strictly speaking, the results will apply only to ADT + Erleada (Apalutamide), but it will still be somewhat relevant for other closely-related lutamides like Nubeqa (Darolutamide).

LIBERTAS - as far as I know the preliminary report is expected this fall or winter and complete finished report 2027 and some parts in 2028 - I think I read that there are several branches in the phase III and not all be finished at the same time.

It depends on what his PSA was prior to starting doublet therapy (ADT + ARPI).

For example - I know a guy whose PSA was 11,700; he’d be thrilled with an ADT-induced PSA of 0.04. In my case, my PSA was only 7.796 and dropped to <0.008 while on ADT alone.

It’s always good when PSA is holding steady or not going up.

What was your husband’s diagnosis (numbers?) going into treatment? What is his testosterone level now? That will define expectations and how your doctor might approach this situation.
