More often than not one knows when something isn't right, but there are things like high blood pressure or hypercortisolism that don't mainifest themselves in any noticeable way. The things that they can cause do show up, often when it's too late. BP is easy to check for. Corisol levels [unless they are too low] rarely get checked until something shows up. My hypercortisolism was found by accident, same with the growths on my kidneys.

No not AFAIK. One could ask fot it, but it may not be covered without a valid reason for doing it.
